Reliant Medical Group, Inc. (“Reliant”) is pleased to offer our patients on-line computer access to portions of their medical record through the MyChart web portal. If you believe that you would benefit from this service, you may sign up for access by completing a request form and agreeing to these Terms and Conditions outlined below:

USE OF MyChart

I understand that MyChart (“MyChart”) should NEVER be used for urgent health care matters. The anticipated turnaround time for a response to a secure message is two (2) business days. Therefore, if I believe I am experiencing an urgent or life threatening health condition I should phone my physician, go directly to the Emergency Department of a local hospital or call 911 immediately.

If my physician is out of the office or unavailable to respond to my secure MyChart message, my message may be routed to another Reliant provider who is part of my care team.

At this time, we anticipate that the MyChart web portal will provide the following services:

  • Ability to review, request or cancel appointments
  • Ability to view selected portions of my medical record such as allergies, medications, histories, problem list and test results
  • Communicate with my physician and healthcare team via secure messaging regarding health concerns, including participating in an e-visit
  • Request prescription refills
  • Links to other sites that will provide general health information

Other services may be added as Reliant enhances the MyChart web portal.

I understand that Reliant providers may send me messages via MyChart. It is my responsibility to monitor these messages and respond in an appropriate manner. I must provide Reliant with a valid email address and update my email address when necessary through “My Profile” while logged into my account. Reliant will send me emails via my personal email address to notify me that I have new MyChart messages. Messages sent via my personal email will not contain personal health information (“PHI”).

I agree that all messages that are sent through MyChart will be regarding MY PERSONAL health care and conditions. I understand that asking for advice for a friend or family member is NOT allowed. Since these messages will become a part of my permanent medical record and will be perceived as my health information, it is important that no one else's information be reflected in my messages. With my authorization, this information may be released to insurance companies when applying for insurance. Reliant takes no responsibility for erroneous medical information that is provided by me to MyChart that may impact my ability to be covered by insurance. Without my authorization, medical information may be released to my insurance for payment of my healthcare claims.

These Terms and Conditions are a binding agreement between me and Reliant. By using or accessing MyChart, I accept and agree to be bound by these Terms. This agreement and use of MyChart shall be governed by and construed in accordance with the laws of the Commonwealth of Massachusetts.


In addition, the following are other prohibited uses and/or activities that may result in termination of MyChart access:

  • Overuse and/or abuse of the MyChart functionality; including (but not limited to) messaging for urgent matters.
  • Inappropriate language;
  • Inappropriately sharing passwords; and
  • Inappropriately including other individual/patient questions/concerns within my medical record.

The above list is not all inclusive and other prohibited uses and/or activities may result in termination of MyChart access at the discretion of Reliant.


Reliant reserves the right to deactivate messaging privileges to MyChart. The following measures will be taken before deactivation:

  • 1st offense: re-education
  • 2nd offense: warning
  • 3rd offense: deactivate messaging privileges and change to view only access


Both Federal and Massachusetts state laws govern the confidentiality of medical information. Reliant is dedicated to keeping medical information confidential. Reliant employees are subject to strict standards on maintaining confidentiality. Since Reliant Medical Group is providing access to medical information I must share this responsibility. Please read Reliant's privacy statement and Notice of Privacy Practices for more information. I recognize that it is my responsibility to ensure the security of my own healthcare information when I access it electronically through MyChart, or when such information is handed to me in printed form, such as an After Visit Summary.


I understand that I will be given a unique access code by Reliant to allow me access to MyChart. The first time I log into MyChart I will be required to create a unique five+ character User ID and an 8+ character alpha-numeric password. Spaces and special characters will not be accepted in the ID or password. I agree to keep my user ID and password confidential. Anyone with access to my user ID and password would have access to my PHI, ability to add to my PHI and message my providers as if they are me. It is my responsibility to change my password if I believe it has been compromised or shared with someone else. It is recommended to change your password every 180 days. Every time I log into MyChart, my user ID and password will be logged in an audit trail in the computer system.

I understand that if Reliant discovers that I have inappropriately shared my password or that I have abused my access privileges, Reliant may terminate my participation in MyChart. I understand that Reliant takes no responsibility for and disclaims any and all liability for damages arising from a breach of medical record confidentiality resulting from my sharing or losing my password.


I understand that MyChart may not always be available to me due to system maintenance, back-ups or failures. Access is provided on an "as available,” basis and I am not guaranteed access at any particular time.


Reliant has taken steps to insure that your personal healthcare information is protected by state of the art electronic security measures, including 128 bit encryption, to prevent unauthorized access to your information through the internet. We also monitor our internet link on a regular basis to detect any inappropriate attempts to access protected health information.


I understand that Reliant may provide links or reference to other sites but RELIANT has no responsibility for the content of such other site and shall not be liable for any damages or injury arising from that content. Any links to other sites are provided as merely a convenience to the users of this web portal.


As a condition of my use of the MyChart, I agree to indemnify and hold Reliant and its Third Party Content providers harmless from and against any and all claims, losses, liability, costs and expenses (including but not limited to attorneys' fees) arising from my use of the MyChart, or from my violation of these Terms and Conditions.


MyChart is protected by applicable copyright laws. Accordingly, I may not copy, distribute, modify, post or frame-in MyChart, including any text, graphics, video, audio, software code, user interface design or logos for or of Reliant.


All content on MyChart is presented only as of the date published or indicated, and may be superseded by subsequent events or for other reasons.


Reliant and you shall defend, indemnify, and hold harmless each other, each other’s officers, employees, and agents from and against any and all third-party claims, liabilities, damages, demands, losses, causes of action and suits, including reasonable attorney’s fees incident thereto, to the extent they result directly from or out of (a) any injury or death of any person or damage to or destruction of any property caused by the negligent acts, errors, omissions or willful misconduct of either party; or (b) any violation of federal or state regulations, orders, rules or the violation of any other governmental entity by either party, its agents or employees; or (c) any breach of warranty or any negligent performance by either party or its obligations under this agreement.

MyChart is made available to you "as is" and "as available" and Reliant Medical Group does not warrant that any defects or inaccuracies will be corrected.

Reliant Medical Group Inc. does not warrant that MyChart will meet my needs, or that they will be uninterrupted, timely, secure or error-free. Reliant also makes no warranty that the results obtained from the use of MyChart will be accurate or reliable, or that the quality of any products, services, information, or other material obtained by me through MyChart will meet my expectations.


Reliant will not be liable to me or anyone else for any consequential, incidental, special, direct, or indirect damages (including but not limited to illness, death or dismemberment that result from use, misuse or loss of use of the MyChart and third party content, inconvenience or delay). This is true even if Reliant has been advised of the possibility of such damages or losses.

Reliant will not be liable to me or anyone else for any loss resulting from a cause over which such Reliant does not have direct control. This includes failure of electronic or mechanical equipment or communications lines (including telephone, cable and internet), unauthorized access, viruses, theft, operator errors, severe or extraordinary weather (including flood, earthquake, or other act of God), fire, war, insurrection, terrorist act, riot, labor dispute and other labor problems, accident, emergency or action of government.


Can I give other people permission to access my MyChart account?

Yes, individuals age 18 or older may designate proxy access to their MyChart account by completing the Adult Proxy Access Request Form at any Reliant Medical Group clinic location. Both the patient and person receiving proxy access must be present together to sign the consent form. A MyChart account will be activated for both the proxy and the account holder. A person may have multiple proxies.

Can I have access to my child’s records?*

Yes, parents or legal guardians of minor children may request access to the minor child’s account by completing the Minor Proxy Access Request Form at the minor’s provider’s office. Such access will only be granted to parties with parental rights or legal guardianship over the MyChart account holder and only to the extent that the party requesting proxy access can demonstrate the legal right to account holder’s medical information. A Proxy for minor children between the ages of 13 and 17 (inclusive) will not be eligible to participate in the full spectrum of MyChart services. Under State and Federal law, there are certain types of medical information that the parent or guardian of a minor patient age 13 or older may not view without consent of the minor patient. Thus, when a minor patient reaches age 13, restrictions will be placed on MyChart access until he/she reaches age 18. At age 18, minor proxy access terminates.

What is an Adolescent MyChart account?*

Minor children between the ages of 13 and 17 (inclusive), with the permission from a parent and/or legal guardian can obtain a MyChart account. The child and the parent and/or legal guardian must both be present in the child’s Providers office to complete the Adolescent MyChart Proxy Request Form and if necessary provide proof of parental rights and/or legal guardianship over the MyChart account holder. The parent and/or legal guardian is required to sign the consent form.


Parents or guardians of minor children may request access to the minor child's account by following the terms outlined in the "Can I have access to my child’s records?*”section of this Terms and Conditions statement. Minor children between the ages of 13 and 17 with permission from a parent and/or legal guardian can obtain their own MyChart account with full access to their information as outlined in the “What is an Adolescent MyChart account?*” section of this Terms and Conditions statement.
