MyChart is a free service offered to Reliant Medical Group patients.

How do I sign up?

Patients who wish to participate will be issued a MyChart activation code during their visit. This code will enable you to login and create your own user ID and password. If you were not issued an activation code, you may visit any Reliant Medical Group site, or you may sign up during your next office visit. Patients ages 18 and above can request an activation code online by clicking on “Request Access Code” in the blue box on the upper right-hand side of this screen.

Can I see my test results in MyChart?

Yes. Test results are found under the “My Medical Record” tab.

Why are certain test results not shared electronically via MyChart?

Your provider is able to determine which types of test results are able to be accessed through MyChart. Further, tests of a very sensitive nature are not released to MyChart.

If some of my health information in MyChart is not correct, what should I do?

Your MyChart information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next visit. Your health information is reviewed and updated in your electronic medical record each visit. You may also send a message about the error to your doctor via the Message function.

If I send a message to my doctor or nurse, when can I expect a reply?

You will generally receive an answer within 2 business days. Please note that MyChart should not be used for urgent situations. Please contact your doctor’s office directly by phone if the situation requires immediate attention or dial 911 if it is an emergency.

How can I ask a question about my test results in MyChart?

In the Message Center select “Request Medical Advice” and
choose "test results" as the subject for the message. Be sure to choose the appropriate provider also.

How can I request a refill of a medication in MyChart?

In the Message Center select “Request an Rx Renewal.” Select the medication you wish to refill and select the pharmacy you would like it to go to. It will default to the pharmacy we have on file for you but you can change it by entering the Pharmacy information in the text field.

How can I request an appointment in MyChart?

In the Message Center select “Request an Appointment.” Select the provider you wish to see, date and time frame, and also include a message in the text field as to why you would like to have an appointment.

How can I cancel an appointment in MyChart?

In the Appointments tab select “Cancel Appointments.” This will show you all scheduled appointments. To cancel, click the check box and click cancel.

Note: some appointments will require you to call and cancel.

How can I ask my doctor a question?

In the Message Center select “Request Medical Advice.” Fill out all the appropriate fields and send.

Note: these messages are sent to the nursing staff at the department and they will forward on to the provider if necessary.

Can I access my entire medical history on MyChart?

No, MyChart does not provide access to your entire health record. If you require your full medical record you can request a copy directly from Reliant Medical Group’s Medical Records Department. What is an E-visit?

An E-visit is an electronic visit completed by a patient and sent through MyChart to his or her Health Care Team. They are perfect for patients who do not have time to get to their doctor’s office or who do not want to spend travel time to visit the doctor when they need a quick answer or diagnosis for a minor symptom.

What symptoms are appropriate for an E-visit?

There are 5 common medical conditions appropriate for an E-visit; Red eye, Cough/cold, Sinus problems, Back pain and Urinary symptoms (female non-diabetic patients under 60 only)

Who is eligible for an E-visit?

E-visits are available to patients that see a Reliant Medical Group Primary Care Physician.

Can I do an E-visit for my child?

No, E-visits are only offered to Adult patients that see one of our Primary Care Physicians. Pediatric patients and specialty patients are not eligible for E-visits.

How do I begin an E-visit?

To initiate a MyChart e-visit, please follow these steps:
1. Log on to your MyChart account.
2. Click on the “Begin an e-visit” box on the main page or under the “Message Center” tab, click on “Begin an e-visit”
3. Read the brief e-visit explanation and then click “accept”
After completing the questionnaire, your E-visit will be reviewed by your physician’s office and you will be offered treatment options. Sometimes your responses may require you to come in for an office visit with your provider.

Can I view a family member's health record in MyChart?

Yes, this is called Proxy access and allows a parent, guardian, or designee to log into their personal MyChart account, and then connect to information regarding their family member.

Can I ask questions regarding a family member from my MyChart account?

No, however you may communicate about another individual’s healthcare information but only if you are their MyChart Proxy. You will need to access their MyChart account through your own personal MyChart account which will be located under the “My Family’s Record’s” tab.

Can my spouse and I share one MyChart account?

No, due to the sensitive nature of medical information, each adult must sign and submit a Release of Information request and establish their own MyChart account.

Can you send me a new access code if I have lost it, let it expire or did not receive it?

In order to obtain an initial access code you will need to come into your doctor’s office. Privacy issues prevent us from sending the initial access code via U.S. Postal Mail or e-mail. However, you may call us for a new code at 774-261-1292 if your initial code has expired or gotten lost.

How is MyChart secure?

We take great care to ensure your health information is kept private and secure. Access to information is controlled through secure access codes, personal ID's, and passwords. Each person controls their password, and the account cannot be accessed without that password. Futher, MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyChart. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.

What is your Privacy Policy?

MyChart is owned and operated by Reliant Medical Group and is fully compliant with federal and state laws pertaining to your privacy. Your name and e-mail address will be treated with the same care and privacy given to the rest of your health records and will never be sold or leased by Reliant Medical Group.

I was automatically logged out of MyChart, what happened?

We aim to protect your privacy and the security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

What do I need in order to use MyChart?

Once your receive your activation code from Reliant Medical Group, you need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer), as well an active user i.d and password.

My access code does not work, what should I do?

For your security, your access code expires after 30 days and is no longer valid after the first time you use it. If you still have problems, email us at mychart@ReliantMedicalGroup.org or you can call 774-261-1292.

Is my access code my user ID?

No, your access code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. (The code will expire after you have used it or after 30 days). When you log into MyChart the first time, you will then be asked to create your own unique MyChart ID and password.

How to access MyChart using AOL?

Please note: This issue has been reported and can be reproduced with AOL 9.5. At this time, there is no known solution. Please follow these instructions as a workaround to using the AOL 9.5 browser.

While logged into AOL, click on the blue 'e' icon in the lower left-hand corner of the screen. It is located next to the Windows button (Windows 7 and Vista) or Start menu button (Windows XP, 2000, etc.). This will open a new Internet Explorer window outside of your AOL workspace.

Type https://mychart.ReliantMedicalGroup.org in the address bar of the Internet Explorer window and press Enter.

Log into MyChart in the Internet Explorer window.

Visit the app store to download the MyChart app. Once downloaded, select Reliant Medical Group as your provider and then enter your personal MyChart ID and password. You must already have an active MyChart account in order to use the iPhone/iPad app.
