Return to TopMyChart is a free service offered to Reliant Medical Group patients.
Return to TopPatients who wish to participate will be issued a MyChart activation code during their visit. This code will enable you to login and create your own user ID and password. If you were not issued an activation code, you may visit any Reliant Medical Group site, or you may sign up during your next office visit. Patients ages 18 and above can request an activation code online by clicking on “Request Access Code” on the MyChart page and selecting the blue box at the bottom of the screen.

Return to TopYour provider is able to determine which types of test results are able to be accessed through MyChart. Further, tests of a very sensitive nature are not released to MyChart.
Return to TopYour MyChart information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next visit. Your health information is reviewed and updated in your electronic medical record each visit. You may also send a message about the error to your doctor via the Message function.
Return to TopYou will generally receive an answer within 2 business days. Please note that MyChart should not be used for urgent situations. Please contact your doctor’s office directly by phone if the situation requires immediate attention or dial 911 if it is an emergency.
Return to TopClick on the Messaging Tab select “Get Medical Advice” or you can click on Send a Message to your Provider’s Office Quick link, click on General Question and choose "test results" as the subject for the message. Be sure to choose the appropriate provider also.
Return to TopClick on the Messaging Tab and select “Request an Rx Renewal” or from the Home Page, you can simply click on the Refill your Medication Quick Link. Select the medication you wish to refill and select the pharmacy you would like it to go to. It will default to the pharmacy we have on file for you but you can change it by entering the Pharmacy information in the text field.
Return to TopClick on the Visits Tab and select “Request an Appointment” or from the Home Page, you can simply click on the “Request an Appointment” Quick Link. Select the provider you wish to see, date and time frame, and also include a message in the text field as to why you would like to have an appointment.
Return to TopClick on the Visits Tab and select “Cancel Appointments.” This will show you all scheduled appointments. To cancel, click the box of the appointment you wish to cancel and click “continue”. Select a “Cancel Reason” and “Comment” if you wish to leave a comment and click “”Confirm cancellation”.
Note: some appointments will require you to call and cancel.
Click on the Message Tab and select “Get Medical Advice” or you can click on Send a Message to your Provider’s Office Quick link. Fill out all the appropriate fields and send.
Note: these messages are sent to the nursing staff at the department and they will forward on to the provider if necessary.
No, MyChart does not provide access to your entire health record. If you require your full medical record you can request a copy directly from Reliant Medical Group’s Medical Records Department. Return to TopAn E-visit is an electronic visit completed by a patient and sent through MyChart to his or her Health Care Team. They are perfect for patients who do not have time to get to their doctor’s office or who do not want to spend travel time to visit the doctor when they need a quick answer or diagnosis for a minor symptom.

Return to TopTo initiate a MyChart e-visit, please follow these steps:
1. Log on to your MyChart account.
2. Click on the Messaging Tab and select “Get Medical Advice”, select “Symptom Specific E Visit”.
3. Read the Terms and Conditions of Use and click “Accept”
After completing the questionnaire, your E-visit will be reviewed by your physician’s office and you will be offered treatment options. Sometimes your responses may require you to come in for an office visit with your provider.
There is no charge or co-pay to use this service at this time
Return to TopYes, this is called Proxy access and allows a parent, guardian, or designee to log into their personal MyChart account, and then connect to information regarding their family member.
Return to TopNo, however you may communicate about another individual’s healthcare information but only if you are their MyChart Proxy. You will need to access their MyChart account through your own personal MyChart account which will be located under the “My Family’s Record’s” tab.

Return to TopIn order to obtain an initial access code you will need to come into your doctor’s office. Privacy issues prevent us from sending the initial access code via U.S. Postal Mail or e-mail. However, you may call us for a new code at 774-261-1292 if your initial code has expired or gotten lost.
Return to TopLog into MyChart and click on the Preferences Tab and select “Security Settings” to change your password or security question. To update your Demographics or Email address, click on the Preferences Tab and select Demographics.
Return to TopTo pay your bill online, please log into your MyChart account. From the Home page, if you have an outstanding balance, you can click on View details for your account. Click on this statement which brings you to Account Details. You can also click on the Billing Tab and select the green “Pay” button.

Each person controls their password, and the account cannot be accessed without that password. Further, MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyChart. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.

Your name and e-mail address will be treated with the same care and privacy given to the rest of your health records and will never be sold or leased by Reliant Medical Group.
Return to TopWe aim to protect your privacy and the security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.
Once you receive your activation code from Reliant Medical Group, you need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer), as well an active user i.d and password.

Return to TopNo, your access code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. (The code will expire after you have used it or after 30 days). When you log into MyChart the first time, you will then be asked to create your own unique MyChart ID and password.
